How Do I Cancel My Enrollment in a Group Class?

We ask that all cancellations be in written form.  Cancellation forms are available at the office.  You may otherwise send an email to info@chicagobluedolphins.com with the following information:

1. Student's Full Name
2. Last Date You Wish to Attend Classes
3. Any Reasons Why Your Cancelling

Please note, if you cancel your enrollment during the first 3-months of your enrollment, you will be assessed a $25 cancellation fee.
